Welcome to Northern New Mexico College Sports Medicine. On this page you will find sections with information directed towards parents, incoming student-athletes, and returning student-athletes. Each section has important information regarding insurance, concussions and our policies, as well as the necessary paperwork required for each student-athlete participating in intercollegiate athletics for Northern. There are several important items that must be read, completed and returned to the Northern New Mexico College Sports Medicine Department. This information will be used to establish your personal and confidential medical file with the Sports Medicine department.
We use SportsWare Online to make easier the acquisition of all information pertinent to the health of student-athletes. Below, you will find instructions and links for the creation and updating of your individual SportsWare Online account. All required forms are listed below, and can be accessed through SportsWare Online. Please read all forms CAREFULLY and complete those noted. These forms are important in medically clearing you for intercollegiate athletic participation. In addition proper and complete insurance information assists us in providing you prompt and appropriate medical care in the event of an illness or injury.
Please note that your participation and any required medical care will be delayed if your current insurance information is not on file.
Without these packets completed you will not be allowed to participate in any team workouts or practices.
